Step 1 of 6 - Information

<!–We hope that the application process is as smooth and simple as possible. If you have any concerns please phone us on 01689 891454. This product can be set up over the phone, or we can easily make any corrections to your online application after it has been submitted.

It is very important that you read the key features carefully as there are significant exit charges unless your withdrawal is for your first property purchase, you are over 60 or it is a terminal illness or death claim.

This single page form gathers all the details needed to process your application and will ask you for:

  • Your Details – Brief personal details to identify you on our systems.
  • Plan Details – Monthly premium and start date.
  • Declaration – An HMRC requirement for all new lifetime ISA applications.

We need the information you provide in order to set up the contract between you and us.
